What is the difference between a health check and optimization?

A health check is the diagnostic. It reviews your instance and tells you what is wrong, how serious each issue is and what remediation would involve. Optimization is doing that remediation work. If you already know what needs fixing, go straight to optimization. If you only know that something is wrong, the health check is the cheaper starting point.

Why has our NetSuite got slower over time?

Usually accumulated customization rather than data volume alone. Common causes are scripts that run on every record save when they only need to run on specific conditions, saved searches returning very large result sets to a dashboard, workflows layered on top of each other over several years, and scripts approaching NetSuite's governance limits. Each was reasonable when added. The combined effect is what you feel.

Will optimization break our existing customizations?

Refactoring carries real risk, which is why the work is done in a sandbox first and tested against your actual processes before anything reaches production. Where a customization is load-bearing but badly built, we will say so and agree the approach with you rather than rewriting it unilaterally.

How often should NetSuite be reviewed?

There is no universal interval. The practical triggers are a significant business change such as a new entity, module or acquisition, a noticeable drop in system responsiveness, or reports starting to disagree with each other. Reviewing after any of those is more useful than reviewing on a fixed calendar.
